Informed Consent Form

The Connecticut Writing Project, an organization that provides professional development for teachers of reading and writing, will be researching teaching and learning in urban schools in order to implement programs that are relevant to urban teachers. From December 2001 through June of 2002, we will be visiting urban schools and interviewing students, teachers, administrators, guidance counselors and other specialists in order to find out how we can better serves the needs of urban teachers.

All of our records will remain confidential and the names of participants will not be included in the report. Participation in the study is voluntary and interviewees may discontinue participation at any time.
